What to Bring

To help with diagnosis and subsequent treatment please bring the following:

  • Referrals  e.g. Veteran’s Affairs (D904 Form), EPC or Care Plan Management Forms or The Podiatry Point Referral from your health professional.  (It is not necessary to have a referral to see the Podiatrist)
  • List of your current medication (including dosage and times taken)
  • Results of any investigations (eg. X-rays or pathology reports)
  • Up to 3 pair of your most commonly worn shoes e.g.  work/sports shoes (especially any shoes causing discomfort)
  • Appropriate clothing

Payment

All fees to be paid at time of consultation unless prior arrangement made.

Cash, cheque, Visa, Mastercard, EFTPOS and HICAPS are accepted.

Please make cheques payable to The Podiatry Point.


Sterilisation

All instruments used in the clinic are packaged and autoclaved in accordance with Australian Standard 4815.

Referrals

Veteran Affairs – Gold Card Holders
It is essential for Veterans’ Affairs patients to have a referral (D904 Form) from their Doctor.  This allows their consultations to be paid by the Health Insurance Commission.

WorkCover
WorkCover patients require a Claim Number from WorkCover and a referral from their Doctor.  This allows their consultations to be paid by WorkCover.

Medicare Eligible Patients
Patients with a chronic condition may be eligible for Medicare rebates under the Enhanced Primary Care Program (EPC) or the Medicare Chronic Disease Management Program.  At the Podiatry Point we recognise the cost of managing a chronic condition & we offer bulk billing for EPC patients.Your Doctor will determine if you qualify for either of these programmes and therefore the Medicare rebate.


Health Insurance

Private Health Insurance funds cover podiatry services under their extras cover.
